Icu Note Template - This is one of my projects i've trying to put together for the 3rd year students who will be rotating through with me anyways. Also, one more intern year trick, review your previous daily note so you only have to read your note to find out stuff like tlc day number, abx day number, etc. See what percent recorded smoking status and offer cessation tx. Wrt cause of death and the like, that all goes on the death certificate, not in the death note and, if you're not the primary team you can/should punt that job. But i can't think of any situation in which either an extra dose or a missed dose is high risk to cause significant complications (this is different than your acute gi bleeds in which it's a treatment and considerable part of the plan). I'm personally of the opinion that an icu note should be a pretty much the same as a medicine note. Continue dialysis as per renal

The s/o part of an icu note is the same as any other note, except a lot of the time the s portion will have to come from the nursing staff, since a lot of the patients are intubated and sedated and can't give you any subjective info. Can do similar for recording aims, sga metabolic labs,etc. So i am working on improving the rounding reports that epic creates in our program which are essentially useless, especially for surgical teams.
